DUBBEL TERMINATION AGREEMENT

By signing below, I am giving my formal written notice for a full termination of my DUBBEL Membership. 

I understand that as per the initial terms and conditions that I have previously agreed to upon starting the program, this termination will not take effect until these 14 days have passed. 

Any unused services cannot be redeemed after the termination has taken place and all missed payments must be paid at the time of termination or they will be forwarded to our collections department. 

Lastly, this cancellation request will also null and void any previous contracts and agreements once approved.
